If you are unable to attend an appointment, we request that you provide at least 24 hours advanced notice to our office so we can meet the needs of another client. Please note, there will be a $50.00 cancellation or no-show fee if it was not canceled within 24 hours. Exceptions are made for cancellations due to illness or an emergency.
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check with your individual insurance provider to verify service coverage.

If your insurance is not listed, please check with your provider to see if they will cover out-of-network services (many do!). We will be glad to help you be able to file a claim with your provider.
